• Make check payable to : OHP Premiums

• Please include your stub or lower portion of your statement

• Remember to write your DMAP (OHP) Medical Care ID Number on the check or money order

 Call toll-free 888-647-2729 (888-OHP-2PAY) to pay by phone.
